Mail order is a convenient alternative to using our secure online shopping cart. In order to utilize our mail order form, you must have Adobe Acrobat Reader installed on your computer.

If you do not have Word Processing Software download the Mail Order Form as Adobe Acrobat Document and print out the page from your browser.  You will need Adobe Acrobat Reader installed on your computer. To place a mail order, please complete the following steps detailed below:

  1. Print the "Nriched Mail Order" form.
  2. Complete Order form with requested information.
  3. Be sure to print name, shipping address, phone number and email address legibly to avoid processing delays.
  4. Orders must be sent to street address. Orders to PO Box will not be processed.
  5. Residents of California add 8.25% sales tax to sub total.
  6. Make cashiers check or money order payable to HEATHER LA BELLA, INC.
  7. Do not send cash or personal checks.
  8. Place mail order form in sealed and stamped envelope.
  9. Mail order form to:

Make cashiers check and money order payable to  HEATHER LA BELLA, INC.
By providing an email address you will receive an order confirmation with 24 hours after receipt of mail order form.

For mail orders to Alaska , Hawaii or Canada , please email accounts@heatherlabella.com to receive USPS Insured Parcel Post shipping and handling rate. Canadian Mail Orders require money order in U.S. funds.
